Do you provide intellectual or developmental disabilities Waiver Testing?

Yes, as a psychological examiner, Daniel Wysocki Ed.S. is qualified to conduct psychological assessments and provide documentation related to waiver eligibility. 

The evaluation prior to surgery is not meant to prevent someone from having a life-changing surgery. It identifies risk-factors to having a successful surgical outcome. There are several psychological factors that could complicate recovery, and we are able to identify and provide guidance on how to overcome these obstacles. The evaluation is an important step in the surgical candidate’s preparation and allows you to receive psychoeducation. After your evaluation you will have new stress management skills, ideas about your mental health, and feel confident in your upcoming surgery. It will consist of an interview and completion of some standardized testing forms.

We strive to provide maximum flexibility and convenience in supporting your successful surgical outcome, regardless of your location. Yes, we can schedule and conduct your pre-surgical bariatric evaluation through telehealth.

Pre-surgical bariatric psychological evaluation coverage varies by insurance. We will check your coverage and obtain a pre-authorization if necessary. Self-pay cost is $450 (consultation, testing, report).
